✨ MINI CREATIVES CLUB – PARTICIPANT WAIVER & RELEASE OF LIABILITY

Parent / Caregiver Waiver & Consent Form

By ticking the box at the time of booking, you (the “Participant”) acknowledge and agree to

the following terms and conditions, which apply to all sessions, events, workshops, and

birthday parties attended with Mini Creatives Club (“the Company”):

1. ASSUMPTION OF RISK

I understand that participation in Mini Creatives Club activities involves inherent risks,

including but not limited to:

● Choking hazards from small parts, sensory materials, or loose items

● Risk of slips, trips, falls, or collisions

● Risk of allergic reactions to food-based, natural, or sensory materials

● Exposure to outdoor environments including weather conditions, uneven ground,

water, dirt, and animals

● Emotional or sensory overwhelm from new or stimulating experiences

I voluntarily accept full responsibility for any risk of injury, loss, damage, or harm to my

child(ren) during participation in any Mini Creatives Club activity.

2. PARENT / CAREGIVER RESPONSIBILITY

I acknowledge that:

● I am solely responsible for supervising my child(ren) at all times

● Mini Creatives Club is not a childcare service

● A parent or guardian must remain present for the entire session

● I am responsible for ensuring my child(ren) engage safely and appropriately during

activities

3. RELEASE OF LIABILITY

I understand and agree that Mini Creatives Club:

● Is not responsible for injury, illness, trauma, or allergic reactions during sessions

● Is not liable for accidents occurring at any location, including but not limited to farms,

private properties, parks, or third-party venues

● Does not control or guarantee the safety of external environments or venue

conditions

I hereby release and hold harmless Mini Creatives Club, its owner, staff, contractors, and

affiliates from any and all claims, liabilities, damages, or demands arising from participation.

4. ALLERGIES & MEDICAL CONDITIONS

● I understand that sessions may include food-based, natural, and sensory materials

● I am responsible for disclosing all allergies, medical conditions, or sensitivities prior to

participation

● I accept full responsibility for monitoring my child(ren)’s health during sessions

Mini Creatives Club does not provide an allergy-free environment.

5. EMERGENCY MEDICAL CARE

● I authorise Mini Creatives Club staff to seek emergency medical assistance if

required

● I understand that I am responsible for any costs associated with medical treatment

● I acknowledge that Mini Creatives Club does not provide medical or emergency care

services

6. OUTDOOR & VENUE ACKNOWLEDGEMENT

I acknowledge that sessions may take place:

● outdoors

● on uneven or natural terrain

● at private venues (including farms, wellness spaces, or private properties)

● around animals or natural elements

I accept all associated risks.

7. PHOTOGRAPHY & MEDIA CONSENT

Please select at booking:

☐ I give permission for my child(ren) to be photographed/filmed for marketing and

promotional purposes.

☐ I do NOT give permission for my child(ren) to be photographed/filmed.

I understand this may include use on social media, website, and promotional materials.

8. AGREEMENT

By ticking the box at the time of booking, I confirm that:

● I have read and understood this Waiver & Release of Liability

● I voluntarily agree to all terms stated above

● This agreement applies to all current and future sessions attended by my child(ren)

with Mini Creatives Club
